심장의 정상 박동은 동방 결절(정맥동/동결절/Sinus node)에서 기인해서 그 자극이 방실계 결절→히스속→좌우 심방실속→푸르키니 망상 섬유(Purkinje’s reticular fibers)→ 심장 자극전도계→ 규칙적으로 심장이 박동한다.
이상자극(異常刺戟)이 동방 결절 이외 부위에서 형성되어 이소성 심장 박동(Ectopic heart beats)이 일어나는 심장 박동을 기외수축 박동이라고 한다.
예정 박동 주기가 다 끝나기 전 심장박동이 더 일찍 수축될 때 기외수축 박동이 생길 수 있다.
기외수축을 조기수축(早期收縮)이라고도 한다. 심장이 조기에 수축될 때 생긴 부정맥을 기회수축 부정맥이라 한다.
기외수축 부정맥은 부정맥(不整脈)의 일종이다.
기외수축 박동이 있을 때 심장이 규칙적으로 박동하다가 갑자기 한두 번 정도 비정상적으로 불규칙하게 박동하기도 한다.

1. 기외수축 부정맥의 증상 징후

기외수축 부정맥은 소아청소년들이나 성인들에게서 가끔 볼 수 있는 비정상적 부정맥의 일종이다.
일반적으로 기외수축 부정맥이 있어도 심장 자체는 이상이 없는 것이 보통이다.
심장자체에 아무 이상이 없이 생긴 기외수축 부정맥은 건강이나 생명에 아무 지장을 주지 않는다.
그렇지만 심장염을 앓을 때, 심장 수술을 받은 후, 디기탈리스제 등 약물 치료를 받을 때, 또는 그 밖에 다른 여러 가지 경우 기외수축 부정맥이 생길 수 있다. 기외수축 부정맥의 원인에 따라 증상이 생길 수 있다.

2. 기외수축 부정맥의 진단

병력, 증상 징후와 진찰소견 등을 종합해서 이 병이 의심되면 심전도 검사 등으로 진단할 수 있다.

3. 기외수축 부정맥의 치료

심장 자체에 아무 이상이 없으면서 생긴 기외수축 부정맥은 조심스럽게 관찰치료를 하면서 기다리면 대부분 자연히 없어진다.
심장 자체에 어떤 병이 있어 생긴 기외수축 부정맥은 원인에 따라 항 부정맥 약물로 치료하든지 이상 자극 기시 부위 제거 치료(Ablation treatment)로 치료한다.
소아청소년들에게서 볼 수 있는 심실 상 빈맥(Supraventricular tachycardia), 윌프-파킨슨-화이트 증후군(Wolf-Parkinson-White syndrome/볼프-파킨슨-화이트 증후군/ WPW 증후군) 등의 부정맥은 이상 자극 기시부 절제수술 치료(Ablation treatment)로 치료한다
그 치료 효과가 90~98%이다(참조 문헌; New York-Presbyterian Children’s Health. Spring 2008).
